There are both PDF and DOC formats. Which one should I select?

PDF documents are generally the best format to select and print because all internet browsers can display them. Word DOC formats may not be compatible with some computers, and they are password protected. You can open our posted DOCs as "read-only" so no formatting changes can occur.

I completed the online appointment request form but have not received a call back. Did you get my message?
